You must have a prior appointment to register and all paperwork must be present. Bring this information with you to the registration appointment:

• Withdrawal form from previous school
• Unofficial transcript
• Immunization Record
• Proof of Address that you live in the Tahoma School District (eg. Utility bill)
• Birth Certificate
• Custody and Control Form if living with someone other than those listed on the birth certificate
